2012 Medical Release Waiver - Any camper who did not enter necessary MEDICAL info during online registration is required to have this form filled out. You can either mail it in, or bring it to the camp.

2012 Individual Registration Form - form to be filled out by individuals that wish to attend one of the camps. However, you can also register online. Note that group and sibling discounts must be submitted via mail-in registrations.

2012 Team Registration Form - form to be filled out by coaches and team representatives if they do NOT want to register online. We strongly encourage all individuals and teams to use the new online team registration. Registering online eliminates all paper forms, notifies coaches by email when a team member has registered, and allows coaches to view which members of his team have registered.
